Thanks for the sharp eyes, but in my code it was definitely 
$_SERVER['PHP_SELF'] with no space. I just did another test and still 
get this:

>Notice :  Undefined index:  PHP_SELF in...

I think I pasted $_SERVER['PHP_SELF '] from php.net, and somehow, 
Safari added a white space.
