Hi, 
   
         The reply you got from jface at mercenarylabs.com is absolutely perfect.
  The only thing  you should remember is that in your PHP code, you should include the following lines ---
   
  $month = $_POST['month'];
$year = $_POST['year'];
   
  etc .......   at the beginning of the code.
   
   
  Here 'month' and 'year' are the values of name attributes  you gave to your HTML form fields.----

Don't forget to add in your opening   tag, as it looks like it's missing.
