NYCPHP Meetup

Hello:
Wonder if you guys can assist me with this.  I have a contact form in html and a handle form in php.  I have used the same forms many times with other sites and it worked ok.  But for some reason, this time when i click on the "Send Message" button  I get a blank screen, I have being over it and still cannot see my error.  I am attaching the forms for your assistance
